2010 Calendar Order Form

We are currently accepting single copy requests for the 2010 Calendar via this form. This year, we have Spanish calendars available as well. You may order a single copy of the English and/or the Spanish calendar. Duplicate orders and orders from customers who have already placed an order for the 2010 Calendar will not be honored.

Please note: Your order may not ship until December 2009. The shipping date is contingent on printing deadlines, which are subject to change.

Why do I need to supply my phone number?

womenshealth.gov will only use your telephone number to contact you if your Calendar is returned as damaged or otherwise undeliverable.

I can't see the form. It says "Thank you for your request ... we can only take one per user."

If you have already submitted a request, you will not be able to submit another one. This form is to request single copies for personal use. If you are an organization requiring multiple copies, please contact our call center at 1-800-994-9662 for assistance.
